The access model for adults who have an intellectual disability in Windsor-Essex was designed through the joint efforts of the broader Windsor-Essex community including service providers, families, self advocates, and government. As a community, it was agreed that a common application form would be considered the intake and assessment method for individuals wishing to access supports and services. All individuals who require new supports, additional supports or changes that cannot be accommodated by the agency currently supporting them, should complete an application form, including, to the best of their ability, the accompanying budget pages. These forms are available through any support provider, including Community Living Windsor, or you can:
Community Living Windsor staff are available to assist families, self-advocates and others with this process. Additional assistance is available from Family Service Windsor-Essex County (519-256-1831) or Windsor Essex Brokerage for Personal Supports (519-966-8094). Once the application form is completed, or an update has been completed, it is to be forwarded to: Process Coordinator The application information is then collated in a private and non-identifying manner and submitted to a neutral Priority Panel. This panel reviews all applications to create a ranked listing of most in need and forwards this to the Ministry of Community and Social Services. Apart from this application process, direct requests for assistance are always welcomed by Community Living Windsor by way of self-referral, family referrals or from other support providers. Priority is given to families and individuals living within the City of Windsor and is based on resource availability.
